Set up your survey so names aren't recorded when collecting responses

If you're creating a survey, poll, questionnaire, or other type of form with Microsoft Forms, you can change a setting so that names aren't recorded when you collect responses.

Set up your form to not record names

  1. In Microsoft Forms, open the form for which you don't want names recorded when collecting responses, and then select More form settings More options button  > Settings.

  2. In the Settings pane under Who can fill out this form, select one of the following:

    • Anyone can respond - Anyone inside or outside of your organization can submit responses to your form or quiz. Responses automatically come in without names recorded.

    • Only people in my organization can respond - Only people that belong to your organization will be able to respond to your form or quiz.

      • Record name - Uncheck this box so names aren't recorded with responses.

        Note: If you have a Microsoft 365 work or school account, the Record name setting is available to you. When unchecked, names won't be recorded with responses. When checked, your form will collect responses with personally identifiable information. If you have a Microsoft account (Hotmail, Live, or Outlook.com), the Record name setting is not available and your form is automatically set to collect responses without names recorded.

        Tip: Under Record name, check One response per person to limit form or quiz responses to only one per person. This will help prevent individuals from filling out a form or quiz multiple times.

    • Specific people in my organization can respond - Only people or groups in your organization that you specify can respond to your form.

      • Record name - Uncheck this box so names aren't recorded with responses..

  3. When ready, you can send out your form and collect responses.

How do I know if my form is not recording names?

When you create your form and uncheck the Record name setting, your intention is to collect responses that don't contain any personally identifiable information from your form responders. While this list is not exhaustive, here are a few examples of different scenarios that could nullify your intention of not recording personally identifiable information, even with the Record name setting unchecked:

  • If one or more questions asks for personally identifiable information (such as a name) and your form responders provide it, names will be recorded.

  • If you send out your form to only a few specific people in your organization, you might be able to match them with their responses depending on what questions are asked and how they're answered.

  • If you send out your form to people that live in different time zones, you might be able to match them with their responses depending on the time stamp (such as "start time" or "completion time") for when responses were submitted.
