Use PowerPoint with your keyboard and a screen reader to insert a form or quiz into a presentation. We have tested it with Narrator, JAWS, and NVDA, but it might work with other screen readers as long as they follow common accessibility standards and techniques. You'll learn how to insert a form or quiz that you've prepared earlier in Microsoft Forms or how to start creating a new form or quiz directly from PowerPoint.

Inserting a form is only available to Office 365 Education and Microsoft 365 Apps for business customers using the PowerPoint for Microsoft 365 version 1807 (Build 16.0.10711.20012) or later on a PC. At this time, inserting a form is not available for PowerPoint for the web.

Insert an existing form or quiz
You can quickly insert an existing form or quiz that you've prepared in Microsoft Forms to a PowerPoint slide.
Insert an existing form or quiz using Narrator

In your PowerPoint presentation, navigate to the slide where you want to insert the form or quiz.

Press Alt+N, F. The Forms pane opens. The focus is on the New Form button.

Press the SR key+Right arrow key until you hear the name of the form or quiz you want to insert, and press Enter.

Press the SR key+Right arrow key until you hear "Insert, menu item," and press Enter. The form or quiz is inserted into the slide.
Insert an existing form or quiz using JAWS

In your PowerPoint presentation, navigate to the slide where you want to insert the form or quiz.

Press Alt+N, F. The Forms pane opens. The focus is on the New Form button.

Make sure that the virtual navigation is on, and then press the SR key+Right arrow key until you hear the name of the form or quiz you want to insert, and press Enter.

Press the Right arrow key until you hear "Insert," and press Enter. The form or quiz is inserted into the slide.
Insert an existing form or quiz using NVDA

In your PowerPoint presentation, navigate to the slide where you want to insert the form or quiz.

Press Alt+N, F. The Forms pane opens. The focus is on the New Form button.

Press the Tab key until you hear the name of the first available form or quiz.

Do one of the following:

To insert the first available form or quiz, press the Right arrow key once. You hear: "Insert, insert form." Press Enter.

To browse the list of available forms and quizzes, switch to virtual navigation, press I repeatedly until you hear the name of the form or quiz you want to insert, and then press Enter. Switch back to focus navigation, and then press the Right arrow key once. You hear: “Insert, Insert form.” Press Enter.
The form or quiz is inserted into the slide.

Create a new form or quiz
You can start creating a new form or quiz straight from the PowerPoint slide where you want to add the form or quiz.

In your PowerPoint presentation, navigate to the slide where you want to add a new form or quiz.

Press Alt+N, F. The Forms pane opens. The focus is on the New Form button.

Do one of the following:

To create a new form, press Enter.

To create a new quiz, press the Tab key until you hear "New quiz button," and press Enter.

Microsoft Forms opens in a web browser. You can now fill in the details and add questions to your form or quiz. For instructions on how to create a form or quiz, refer to Use a screen reader to create a new form in Microsoft Forms or Use a screen reader to create a new quiz in Microsoft Forms.

Once you're done creating the form or quiz in Microsoft Forms, return to the PowerPoint slide. The new form or quiz has been added to the slide.
